Any Indian citizen having 40% or more disability under the 21 specified categories of the Rights of Persons with Disabilities (RPwD) Act 2016.

How it works (Step-by-Step)
- 1Visit swavlambancard.gov.in and click 'Apply for Disability Certificate & UDID Card'.
- 2Fill in personal details, educational background, employment status, and disability specifics.
- 3Upload photo, signature, and address proof, then choose your nearest designated Government Civil Hospital.
- 4Attend the scheduled Medical Board assessment at the civil hospital on the allocated date.
- 5Upon approval by the Medical Specialist, download the digital e-UDID card and receive the plastic smart card by speed post.
