Serving Central Government employees, Central Civil/Defence pensioners, Members of Parliament, sitting/retired Supreme Court/High Court judges, and eligible dependents.

• 1Proof of Age and Identity (Aadhaar Card / PAN Card)
• 2Last Pay Certificate (for serving staff) or PPO Pension Payment Order (for pensioners)
• 3Proof of Residence / Quarter allotment order
• 4Passport-size family photographs
• 5Disability certificate / Dependency certificate (if applicable for dependent children)

   Visit the official CGHS portal at cghs.nic.in and click on 'Apply for Plastic Card'.

   Fill in employee/pensioner service details, ministry/department, and dependent family member details.

   Upload photograph, signature, PPO/pay slip, and residential address proof.

   Pay the subscription fee via Bharatkosh (for pensioners) or payroll deduction (for serving staff).

   Download the provisional index card immediately and collect the plastic card from your designated CGHS Wellness Centre.
